CHA2-DS2-VASc score tool is used to calculate the risk of having stroke each passing year on patients with Atrial Fibrillation. It is composed of medical conditions and demographic factors that generally increase the risk of cerebrovascular accident.

CHA2-DS2-VASc score is a mnemonic which stands for the above mentioned medical conditions.
If the risk of stroke is 2% or higher (score of 3 or more), Anticoagulants like warfarin should be given.
Warfarin dose is adjusted to maintain INR range between 2 to 3:
- Warfarin 5mg po daily as a starting dose.
- Blood INR level should be checked every 48 hours until therapeutic range of 2 to 3 is reached.
All patients on lifelong warfarin should be assessed for risk of bleeding using Has-Bled Score.
